About the Provider

Hello there!

I am Courtney Forman, a Double Board Certified Advanced Practice Nurse with pending Full Practice Authority. 

I am the owner of THRIVE Mental Wellness. 

My nursing and medical backgrounds include Adult Progressive Care, Neonatal and Pediatric ICU, General Pediatric inpatient care, and Family Practice. My psychiatric background includes long term care facility and geriatric psych, significant outpatient psychiatric management, and inpatient psychiatric consultations. I am particularly well skilled in managing pediatric psychiatric conditions, and depression and anxiety in women. I see all ages. I have been working in healthcare for over 16 years. 

I am committed to excellent care that includes more than just a prescription, I want to see my patients THRIVE. 

I believe the best care management plans are comprehensive for each patient. I complete in depth initial consultations and allow time to gather all pertinent patient and family history, including lab work, genetic testing and hormone levels.. 

I work with each patient and family to create a comprehensive plan based on their unique goals.
